It's pronounced Vee-back – Vaginal Birth After Cesarean – When a woman has a cesarean birth to a baby and then have a baby vaginally later. Sounds simple enough, does not it? Not in this political climate and health, it is not.
Forty years ago, the rule was "Once a cesarean, always cesarean section. "In the 1980's, a swing occurred when obstetricians (OB – physicians who specialize in pregnancy and childbirth) began to support VBAC. Now the pendulum swings (many believe has tilted) away to the latest statistics that show the cesarean rate in the U.S. is 31%. Given the World Health Organization says the rate of caesarean sections should not exceed 15%, our 31% is outrageous.
Women who desire a VBAC may be looking long and hard to find a vendor to provide their birth. In many parts of the country every hospital and every doctor in the area refuse to do a trial of labor (TOL) resulting in a vaginal delivery if successful. Many factors contribute to this negative administrative requirements including an obstetrician and anesthesiologist in-house as VBAC women work. This may be cost prohibited doctors, and that would allow them to office hours or spend time with their families. Another reason not to VBAC is malpractice insurance, the doctor will not let them, they feel they are simply too risky.
What are the risks of vaginal birth? A percentage very small women have a uterine rupture is 0.8%. Women who have a uterine rupture can have catastrophic consequences as the baby's death as the mother of bleeding. Very, very rarely, a mother can bleed to death from the opening of the uterus. However, when the person is, the risk is 100%. Therefore, some doctors refuse to do VBAC.
PVDC is recommended not be induced (by oxytocin or prostaglandins), and that seems to cause many of uterine rupture. The epidural is not seen as a cause of a breakdown, however, that women need not go without pain medication. However, highly encourage supporters of PVDC natural childbirth as the rupture can often be felt early enough to avoid the tragic consequences of a complete rupture.
