Gastroesophageal reflux disease (GERD) is a very common complaint in clinical practice. According to the Merck Manual online, "the incompetence of lower esophageal sphincter allows reflux of gastric contents into the esophagus, causing burning. Prolonged reflux may lead to esophagitis, stricture, and rarely metaplasia or malignancy. The diagnosis is clinical, sometimes with endoscopy, with or without acid testing. Treatment involves lifestyle modification, deletion acid inhibitors, proton pump, and sometimes surgical repair. Gastroesophageal reflux disease (GERD) is common and occurs in 30 to 40% adults. It also occurs frequently in infants, usually from birth. "
Complications of this disorder include esophagitis, ulcer esophageal ulcer, esophageal stricture, Barrett's esophagitis or esophageal adenocarcinoma. The main symptom of GERD is heartburn, with or without regurgitation acid in the mouth that may be accompanied by vomiting, irritability, anorexia and sometimes chronic aspiration (for children) and cough, hoarseness or wheezing (in adults with chronic aspiration). Treatment strategies in Western medicine include raising the head of the bed, avoid coffee, alcohol, fats and smoking and drug inhibition of proton pump inhibitors (such as omeprazole, lansoprazole, esomeprazole, metoclopramide, ranitidine) .1
In the case of GERD (At least the kind with the symptom of acid regurgitation) who have a rare 1:01 allopathic correlation of disease description and category Chinese disease. The Tun Suan "acid regurgitation" disease category of Chinese medicine is that GERD is naturally at home. According Sionneau and gangs in the treatment of disease in TCM, there are three differentiation model to consider in the treatment of GERD in Chinese medicine. The patterns are:
1. Liver fire invading the stomach (early treatment: liver clear and harmonize the stomach formula: modified Zuo Jin Wan; acupuncture points: CV-13, CV-11, LV-2, E-34CV-12, ST 36, UB-17, PC-6, ST-44)
2. Moisture Internal cold block (early treatment: rectify the qi, transform dampness, and harmonize the center along with the heat off; formula acupuncture points and modified Xiang Sha Liu Jun Zi Tang: CV-13, CV-12, CV-10, SP-9, E-34, CV-17, PC -6, ST-36, TB-6)
3. Accumulation and stagnation of food in the stomach (Principles of treatment: to harmonize the center and the stagnation is scattered; formula: The modified Bao Wan, acupuncture points: CV-21, ST-20, CV-12, ST-36, SP-4, ST-25, TB-6, PC-6) 2
I usually mine patients to consider the approach Chinese medicine for the treatment of this disease to avoid the side effects of inhibitors of proton pump, which include headache, diarrhea, constipation, abdominal pain, nausea and rash.
