Polycystic ovary syndrome is an endocrine disorder in general with predominant manifestation in female reproduction. It is diagnosed if two the following conditions are present

1. Top of the male hormone testosterone, detected by the blood level or the presence of hirsutism or excessive hair acne.

2. Anovulation, the absence of ovulation in most women's cycles

3. polycystic appearance. On ultrasound many small follicles is within the outer capsule of the ovary.

No other tests are required for diagnosis. Because some conditions may be wrongly classified as PCOS, doctors can measure other hormones to differentiate PCOS from other conditions. Due to its general effect on blood sugar to insulin and lipids, many doctors also measure glucose, insulin, cholesterol and other lipids.

Women who do not want to get pregnant immediately should be placed on oral contraceptive pills or progesterone synthetic drugs to antagonize the effects of estrogen. Prolonged exposure to estrogen in patients with PCOS may lead to the proliferation of the lining of the uterus, endometrial hyperplasia which may progress to cancer endometrium.

Women who want to become pregnant should be evaluated by male factors, tubes and uterus. Male factor is evaluated by taking a complete medical history and semen analysis. Tubal factor was assessed using a hysterosalpingogram, a dye proof of the uterine cavity and fallopian tubes. Once the fallopian tubes, other factors are excluded from patentability, normal uterine cavity and normal semen analysis, attention is directed toward the induction of ovulation. The induction of ovulation is often combined with intrauterine insemination, IUI time. This will ensure that the proper amount of active sperm are present in the fallopian tubes at the time of ovulation.

Ovulation Induction. Before any treatment nutrient management advice is prudent. Overweight women should be encouraged to lose weight and begin a proper exercise program. Weight loss can lead to the resumption of ovulation. The aim of ovulation induction since then is to produce small number follicles (one to two) to reduce the risk of multiple pregnancy. Multiple pregnancy is associated with risks to the health of the mother and preterm labor and long-term consequences for the fetus sometimes. For this reason, the use of these drugs should be monitored by ultrasound and sometimes blood. Do not take any of these without supervision to avoid multiple ovulation and multiple pregnancy.

1. Clomiphene citrate-Clomid. Oral medication used to increase the release of a hormone signal the master gland of the brain to induce the development of follicles, the sac containing the egg, ovaries. The risk of twins is about 8% and triplets or more is 1%. The probability of pregnancy is 10% per treatment cycle. Many doctors prescribe metformin Clomid. This medicine is not proven further benefit in terms of ovulation and should not be used solely for this reason.

2. Aromatase inhibitors, letrozole and anastrozole. These are oral medications also induce the release of the hormone signal the master gland, which then stimulate follicles in the ovaries to grow. Some women who do not respond to clomid respond to these drugs. Letrozole was charged with increasing birth defects, but this was later disproved.

3. Surgery. This is achieved using minimal access laparoscopic surgery days, in the inner Ovarian burning electricity use to reduce androgen production. Risks include scarring and reduction of remaining follicles in the ovary. Its use is restricted to a few select cases.

4. - 'Injectable' Drug gonadotropins. These are synthetic hormones similar the hormones of the signal produced by the master glands. They are much stronger than oral medications. Its associated with about 15% pregnancy rate per cycle of treatment, but also at higher risk of multiple pregnancy approximately 30% twins and 3.1% triplets or higher order. Also the risk of ovarian hyperstimulation syndrome is much higher. In this condition the ovaries enlarge and leak fluid into the peritoneal cavity, which makes the blood thicker and more prone to clots. Its use should be strictly monitored by a reproductive endocrinologist and fertility specialist.

5. IVF in vitro fertilization. Ovaries stimulated with gonadotropins. The eggs are retrieved using a needle and a vaginal ultrasound. The eggs are fertilized in the laboratory. Embryos are observed and the best one or two embryos are transferred to the uterus. Although more complicated than ovulation induction and IUI takes advantage of the highest pregnancy rates 30 to 50% depending of age, control the number of embryos transferred to avoid multiple pregnancy and the freezing of excess embryos for use in later years. Fertilization in vitro allows the placement of an embryo in the uterus, virtually eliminating multiple pregnancies and triplets and twins in the reduction of around 1%.

A large study compared step approach using oral medications then then IVF versus injectable drugs orally followed by IVF if pregnancy does not occur, found that the use of oral medication followed by an in vitro fertilization more practical the safest approach and leads to less time to conceive that the gradual approach.
